Dosage/Direction for Use:

Adult & Childs ≥12 years 5 mg once daily; 6-12 years 5 mg once daily
Adult & Childs ≥12 years with mild renal impairment (CrCl 50-80 mL/min) 5 mg once daily; moderate renal impairment (CrCl 30-50 mL/min) 5 mg once every other day; severe renal impairment (CrCl 10-30 mL/min) 5 mg twice weekly (once every 3-4 days)
